This document is the informed consent for licensed healthcare providers to participate as a site in the IRB-approved observational registry operated by the Mortensen Medical Research Network. By signing, the provider agrees to the terms below for the duration of their participation.

1. Purpose of the registry

The Mortensen Medical Research Network operates an IRB-approved observational registry to systematically collect real-world data on the safety, tolerability and clinical outcomes of peptide therapies prescribed by participating clinicians within their normal scope of practice. The registry does not dictate prescribing decisions. It documents them.

2. Provider responsibilities

As a site participant, the undersigned provider agrees to:

  • Prescribe peptide therapies only within the scope of their state license and clinical judgment.
  • Obtain informed consent from each patient enrolled in the observational protocol, using the registry's approved consent template.
  • Report adverse events to the registry within 24 hours of becoming aware of a serious adverse event, and within 7 days for non-serious events.
  • Maintain accurate, timely records of patient outcomes at the intervals specified by the protocol.
  • Source all peptide products exclusively from Peptide Pure LLC or other 503A/503B licensed compounding pharmacies approved by the IRB.

3. Patient protections

Patients enrolled by the provider are protected under the IRB-approved protocol, which includes:

  • Voluntary informed consent documenting the investigational nature of the therapy.
  • The right to withdraw from the registry at any time without affecting continued care.
  • Confidentiality of all data collected, handled in accordance with HIPAA and 21 CFR Part 11.
  • Access to the provider's adverse event reporting channel for any safety concerns.

4. Data ownership and use

De-identified data collected through the registry may be used by the Mortensen Medical Research Network and its collaborators for publication in peer-reviewed journals and scientific conferences, regulatory submissions intended to support the legitimate clinical use of peptide therapies, aggregated safety and efficacy reporting to participating providers, and quality improvement of the registry's protocols and consent procedures.

Providers retain rights to their own clinical records. The registry does not assert ownership over the provider's patient data outside the scope of what is submitted to the registry.

5. Non-circumvention

The provider agrees not to bypass the registry framework by sourcing peptide products from unlicensed or grey-market suppliers while representing treatments as part of the registry, or by directing patients to third-party clinics for the purpose of avoiding the registry's documentation requirements.

6. Term and termination

Participation continues for the duration of the IRB-approved protocol or until either party gives 30 days' written notice of withdrawal. Upon withdrawal, the provider retains access to patient data they submitted and remains bound by confidentiality obligations for data obtained through the registry.

7. Regulatory and legal acknowledgement

The provider acknowledges that the peptide therapies discussed in the registry are not FDA-approved for the indications studied, that participation in the registry does not constitute FDA approval, endorsement or validation of these therapies, and that the provider remains solely responsible for clinical decision-making and for compliance with applicable state and federal laws, including DEA regulations where relevant.
